Arizona CNA Reciprocity


It is important to know that state regulations change often.  This site is updated frequently, however, please double check all information with your states official government agency.  Links and contact information are provided for your convenience.

If an approved CNA moves to Arizona from their home state, applicant must contact the Arizona Nursing Assistant Registry to obtain a transfer/reciprocity form. Completed form with required information, documentations and fees must be submitted by mail. These requirements are (1) proof that he or she is listed as active on nursing assistant activity and (2) proof that he or she is currently working as CNA or completed the state training program and passed the CNA state exam.
