If an approved CNA wishes to work in Oregon they must first apply for endorsement. The applicant must obtain Endorsement form from OSBN.
Applicant must meet the following requirements:

Currently or formerly certified in good standing in a U.S. state or territory

Have completed a nurse aide/nursing assistant training program that met OBRA standards. (Send a copy of certificate of completion–see the CNA Endorsement Application Packet for details.)

Have at least 400 hours of paid employment as a Certified Nursing Assistant in that state in the last two years and was under the supervision of a nurse. Exceptions: This is waived if applicants have been certified less than two years.

Complete a national criminal background check.
All sections of the Endorsement Application Packet must be completed and must be mailed to the address indicated in the form. However, before sending the Endorsement Application, applicant must send FIRST the Verification of Certification Form. Applicant must:

Contact his or her home state.

Check if certification is current or expired. If it is expired, applicant is not eligible for endorsement.

Inquire about the fee for written verification of certification.

Mail the Verification of Certification form with required fee (if any) to applicant’s home state.
Applicant’s home state in which you are certified will return the completed form directly to Oregon State Board of Nursing. Note: There will be a delay if applicant will send the Verificaion Form to Oregon State Board of Nursing. Just let the State in which you are certified with will send the form to OSBN.
