If an approved CNA moves to Tennessee from their home state, applicant must contact the TN Nurse Aide Registry to obtain Verification of Out-Of-State Nurse Aide Registration Application. Individual must have an active certification with no abuse or other derogatory information listed. Completed form with required documents such as a copy of the individual’s social security card and identification, i.e., driver’s license, passport, or other photo ID, etc. must be mailed to the address indicated in the form.
NOTE: Tennessee does not require a processing fee, however, the registry in the state where you are currently listed may require a processing fee.
