If an approved LNA wishes to work in Vermont they must first apply for endorsement. The applicant must contact the
Vermont Licensed Nursing Assistant Registry to obtain an Endorsement Form. The applicant must have an active certification with no abuse or other derogatory information listed and must be listed as good standing in his/her home state. Completed form with required documents must be mailed to the address indicated in the form.
